A PDF file should be downloaded to your computer. You will need to print this form out. Follow the instructions closely.
You will need to pay $50.00. This is the State Board Fee.
Note: The Certificate of Completion form is a roster that confirms you have graduated from nursing school. Your school will send this for you, so you don't need to do anything
This site and Pearson Vue recommends all candidates to read the NCLEX bulletin before registering and taking the test. It can be found on the Pearson Vue NCLEX Homepage
Pearson Vue is the company that hosts the NCLEX. They run the testing centers and the computerized testing on a national level for the NCSBN. The NCSBN is the one who creates the NCLEX content, not Pearson Vue.
Pearson Vue is also responsible for sending the Authorization to Test letter. They will only send it after receiving an approval from your State Board of Nursing (which you applied to in Step 1).
The registration fee will be $200.
Register Online with Pearson Vue here.
Note: When applying online, make sure you choose the the correct RN or PN option! Also, providing an email is required. All communication from Pearson Vue comes to your e-mail, including your Authorization to Test (ATT).
Call NCLEX Candidate Services (Pearson VUE): 1.866.49NCLEX (1.866.496.2539) Monday-Friday, 7 am to 7 pm, U.S. Central Time.
You've successfully completed the dual application process. Your ATT will arrive by e-mail approximately 4-8 weeks after you register. It may feel like a very long wait.
Remember, after you turn in all the materials, your State Board of Nursing (from Step 1) sends approval to Pearson Vue (from Step 2). Pearson Vue then sends the Authorization to Test. You can check the status of both applications.
Check the status of your $200 registration with Pearson Vue's NCLEX Candidate Services at 1.866.49NCLEX (1.866.496.2539) Monday-Friday, 7 am to 7 pm, U.S. Central Time.
The IPLA can be contacted by phone at 317-234-2043 (8:00am-12:00pm M-F) or emailed at pla2@pla.IN.gov.
Print your ATT out! You'll need to bring it (and a form of picture ID) when you take the NCLEX. Electronic copies won't be accepted. If you arrive without these materials, you will be turned away and will have to reregister and repay the examination fee of $200.
Once you've received it, you must take your test within an average of 90 days (depending on your State).
Once your Authorization to Test letter arrives, you can schedule your test from the Pearson Vue website.
